These stay in place perfectly. The extra padding isn’t really noticeable to me. I wear these every day for hours, and they’ve lasted me over a year without showing the slightest bit of wear.

GREAT FOR NON-DANCERS TOO
I love these! The elastic fabric attached to the spacers keep them in place. My mother-in-law loves them and she's never been a dancer!

MY FEET DON'T HURT ANYMORE!
I love these things, they keep my big toe aligned in my tapered box while I'm on pointe and my joints in my feet no longer hurt while dancing or once I take my pointe shoes off after class. I was a bit confused when I got them because they were HUGE. In the picture they didn't seem all that big, I thought they would fit on my pinky toe and all the others in between. But, that is not the case - they are meant to go on your big toe, so if you are looking for something to separate your other toes, you will need something a bit smaller. Otherwise, these are pretty comfortable in the shoes, you just have to get used to the extra padding.
